Immunization

KINDERGARTEN, 1st 2nd, 3rd and 4th GRADE

POLIO: 4 doses or 3 doses if 3rd dose is after 4th birthday (4 doses total)

DTP: 5 doses total or 4 doses if 4th dose is after 4th birthday (5 doses total)

MMR: 2 doses, both after 1st birthday

HEPATITIS B: 3 doses total

VARICELLA: 1 dose or health care provider-documented disease or immunity.

5th and 6th GRADE REQUIREMENTS:

POLIO: 4 doses total or 3 doses if 3rd dose is after 4th birthday (4 doses total)

DTP: 5 doses total or 4 doses if 4th dose are after 4th birthday (5 doses total)

MMR: 2 doses both after 1st birthday

HEPATITIS B: 3 doses total

7th and 8th GRADE REQUIREMENTS:

POLIO: 4 doses total or 3 doses if 3rd dose is after 2nd birthday

DTP: 4 doses, with last dose after 2nd birthday (may require 5th dose)

MMR: 2 doses both after 1st birthday

Hepatitis B: 3 doses total

Immunizations- It is the law: Health and Safety Code Sections 1203525-120380; California Administrative Code, Title 17, Sections 6000-6075.

Students must meet immunizations requirements to attend school. Parents must bring immunization records to school for school to copy. If for personal or religious beliefs parents decide not to immunize a waiver must be signed and filed with the school. What are the immunization requirements?

First Grade Physicals/CHDP’s - It is the law: Health and Safety Code Section 323.5

Students entering first grade must have a physical examination on file with the school.

The examination must have been completed within the prior 18 months of entrance into

the first grade and before completing 90 days in the first grade.
